• New Jersey school immunization rules, Immunization of Pupils in Schools (N.J.A.C. 8:57-4) require students to receive a series of immunizations prior to attendance at school. In addition, schools are required to enforce requirements, maintain records, and submit annual reports to the state and their respective local health department. Schools are audited by the Department of Health to ensure compliance. 

    Preschool Requirements

    • 4 Diphtheria, Tetanus toxoid and Pertussis (DTP/DTaP)
    • 3 Polio
    • 1 Measles, Mumps, Rubella (MMR)
    • 3 Hepatitis B
    • 1 Varicella administered after the 1st birthday
    • 1 Haemophilis (HIB) administered after the 1st birthday
    • 1 Pneumococcal Conjugate administered after the 1st birthday 
    • Influenza vaccine adminstered annually between September 1st and December 31st 

     

    Kindergarten Requirements (students already enrolled in preschool will need to recieve a DTAP and Polio booster)

    • 5 Diphtheria, Tetanus toxoid and Pertussis (DTP/DTaP) (or 4 with one given 
    • after the 4th birthday)
    • 4 Polio (or 3 with one given after the 4th birthday)
    • 2 Measles, Mumps, Rubella (MMR)
    • 3 Hepatitis B
    • 1 Varicella given after the 1st birthday

     

    1st Grade Through 5th Grade Requirements

    Same as Kindergarten

    6th Grade Through 12th Grade Requirements

    Same as Kindergarten with the following due at the beginning of the school year in 6th grade: 

    • Tdap vaccine
    • Meningococcal vaccine
